Event Summary
At the above stated time, we began receiving reports of call failures throughout the network. The issues were determined to be isolated within the Inteliquent/Onvoy network. Inbound calls from those networks were not acknowledging 200 OK messages which are normally sent when a party has answered the call. The carrier immediately escalated the issue and was able to resolve it.

Total disruption was 44 minutes and 45 seconds.
Root Cause Analysis
Onvoy has yet to provide an official RCA. As soon as they provide one we will update this document.
Future Preventative Action
While Onvoy has not provided their RCA we will continue to use a large group of providers in order to limit the potential for widespread outages.
